• Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used: Countertops in rough repair. Cutting boards heavily scored and stained.
• Warewashing facilities, installed, maintained, used, test strips: No drain boards on 3 comp sink. No test strips available for chlorine sanitizer.
• Nonfood-contact surfaces clean: High-touch and low-tough surfaces heavily soiled with dirt and old food debris.
• Plumbing installed; proper backflow devices: 3 comp sink faucet leaking.
• Physical facilities installed, maintained, and clean: Floors, walls, and ceilings in bad repair throughout building.
