What the inspector found

• 2-2A: .03(2)(o)Person-in-charge (PIC) could not demonstrate in a verifiable manner that all employees were informed of their responsibility to report symptoms and diseases.
• 4-1A: .04(4)(c)1(i)(ii)(iii)(v) (vi)(vii)(viii)Observed unwashed broccoli stored overed cut and washed mushrooms, raw pork stored over washed and cut mushrooms in the walk-in cooler. Observed raw chicken stored over raw beef, pork and cooked mussels, and raw beef on top of cooked mussels in the walk-in freezer. Observed raw chicken stored over raw beef and pork, and raw chicken over vegetables in the chest freezer at the front of the facility.
• 12A: .04(4)(q)Observed buckets of raw beef stored directly on the floor of the walk-in cooler and freezer.
• 14A: .04(4)(k)Observed a rice scoop stored in water in a small crockpot temping below 135F.
• 14B: .05(10)(e) 1,2,4Observed several working containers stored as clean but stacked wet on a rack above the dish pit area.
