- Insects, Rodents and Animals not Present: 6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed fruit flys (more then 7) near mop sink and throughout the facility. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem
- Plumbing Installed; Proper Backflow Devices: PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines||Addl Notes: Observed hose with sprayer nozzle attached downstream of AVB valve on mop sink. Informed PIC that shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. P
