• 2-201.11 Provide an employee health policy that states what actions are necessary when an employee has a specific symptom or diagnosis related to diseases that are transmissible through food. Communication of the policy to employees must be verifiable.// No health policy provided for employees during inspection. COS PIC Employees signed the Health Policy before the end of inspection.
• 4-601.11 Clean equipment food contact surfaces and utensils to sight and touch. (choppers, slicers, mixers, utensils in storage)// Slicer blades sitting on clean dish shelving soiled with food debris. COS PIC Voluntarily placed in 3 comp sink for wash rinse and sanitize during inspection.
• 5-205.15(B) Repair condensate leak in refrigeration units. // Accumulation of ice on pipe in WI freezer.
• 6-301.12 Provide paper towels or a continuous towel system that supplies the user with a clean towel at each handwashing sink or group of adjacent sinks.// No paper towels at handwashing sink in the kitchen area. COS PIC Voluntarily replaced paper towels.
• 6-501.11 Maintain physical facilities in good repair (floors, walls, ceilings, ceiling tiles, attached fixtures, etc.).// Repair/Replace damaged ceiling tiles that have a hole in ceiling.
